We have an excellent opportunity for a qualified Social Worker to join the Post Discharge Assessment Team (PDAT) based at the Shay Stadium in Halifax. The team supports adults from the age of eighteen with Care Act Assessments completed for those who have recently been discharged from hospital to either a return home with home care support or to a short-stay care home placement whilst awaiting a further period of assessment.
Applications from Newly Qualified Social Workers are also encouraged and you will be supported to complete your assessed year in practice ASYE. The successful candidate will be joining a re-structured service that is dedicated to strengths-based and outcome-focused practice.

Our social work practice in Calderdale is:
• Human rights based with a key emphasis on the Mental Capacity Act 2005 and subsequent Deprivation of Liberty Safeguards.
• Underpinned by values of social justice, the idea of fairness between the individual and society.
• Ensures individual rights to access health funded services is assessed under the NHS Continuing Health Care Framework (amended in 2018).
• Person-centred when approaching the safeguarding of a vulnerable adult.
• Positive about enabling ‘risk’ considering prevention and the management of risk.
You will be supported in your professional role to:
• Develop multi-agency links while increasing confidence in leading multi-disciplinary meetings i.e., health, social, housing, police, therapy
• Complete legal reports understanding current legislative frameworks and your responsibilities
• Undertake safeguarding duties under s42 of The Care Act 2014
• Complete statutory assessments, understanding eligibility and support planning which outcome is focused.
• Work with carers, considering eligible needs and support planning
The role requires you:
• Take accountability for self-development
• Engage in critical reflective practice, both in 1-1 supervision and group reflect sessions.
• Take ownership of continued professional development needs mapped against the BASW Professional Capabilities Framework (PCF).

As this role is based in a community team driving is an essential requirement of the post.
