  • Undertake holistic assessments early in the transition process to ensure a smooth and successful transfer into Adult Social Care

The Team

The Transition Team consists of 4.5 FTE Social Workers. You will be joining a motivated, supportive, and forward‑thinking team where supervision, development, and collaboration are priorities.

  • Be able to undertake standard social care duties, including:
    • Chairing meetings
    • Providing support and advice to junior staff
    • Producing high‑quality, person‑led assessments and reports (including court reports)

A full driving licence is essential, as the role covers the whole Wakefield District and some young people may be placed out of area.
