This role is an exciting opportunity to join the Supported Accommodation Assessment Team (SAAT) working with adults across North and South Kirklees care homes, supported living accommodation and extra care housing.

Team Details:

SAAT is a new substantive team within Kirklees Adult Social Care that has been developed from an established care home team, and you would be joining us at a crucial and exciting time.

In the SAAT team, you will primarily be working with adults living within 24-hour care home settings, supported living accommodation and extra care housing. You will be required to hold a caseload, complete a wide range of holistic assessments and reviews of a person’s needs under the Care Act and will also be on team rotas for duty and Safeguarding. Throughout, you will be seeking and encouraging the voice and views of the person, their carers, representatives and/or relatives, adopting a strength based and person centred approach. The adults will have social care needs of varying complexity across older people, physical disability, learning disability and mental health, therefore this role is a great opportunity to develop in your career within a supportive and flexible team.

This role also gives the opportunity to build ongoing professional relationships with care staff and other professionals across health and social care including active participation within Continuing Health Care meetings.
