We are seeking motivated and enthusiastic individuals to join our Hospital and Post Discharge Assessment Service. This is an exciting opportunity to work across two dynamic teams that play a vital role in supporting people through their discharge and recovery journey. Depending on service demand, the successful applicant may work within either the Hospital Discharge Team or the Post Discharge Assessment Team, providing a varied and rewarding experience.
The Hospital Discharge Team supports effective, safe and person-centred discharge planning from hospital settings. The team works across Calderdale Royal Hospital, Huddersfield Royal Infirmary and Intermediate Care services, undertaking strengths-based assessments and working closely with health colleagues to ensure people can leave hospital at the right time with the right support in place.
The Post Discharge Assessment Team provides short-term intervention and follow-up support once individuals have returned home. The team completes ongoing assessments and reviews, helping people maximise their independence, achieve their desired outcomes and access the support they need following discharge.

We strive to work collaboratively with our health partners and other agencies to provide a seamless experience for the people of Calderdale and support them to live as independently as possible.
We are seeking social work qualified individuals who are passionate about Adult Social Care, committed to providing high-quality services and keen to develop their professional knowledge and experience. You will play a key role in helping the Council fulfil its statutory responsibilities under the Care Act and Health and Care Act 2022, while promoting wellbeing, independence, choice and control. The successful applicant will work closely with colleagues and partner agencies to safeguard adults and support their right to live in safety, free from abuse and neglect.
